Dear Trainee:

Welcome to the New Mexico VA Health Care System. We are pleased to provide you with an experiential rotation that will develop your knowledge and skills. To provide you with this opportunity, the VA requires several forms, personal background check and training. Please follow the instructions below
    
         1. Complete and Turn in the following forms to your Point of Contact (POC)

        2. HR will contact you to complete background check and you will need to be fingerprinted.

          a. Bring one photo ID and another form of ID. For example:

      1. Driver’s license
      2. Passport
      3. Social security card
      4. Student ID
       3. Bring your VA appointment letter to your POC.
         
       4. Complete on-line training

5. Call PIV office at (505) 265-1711 ext.: 7370 to set up appointment to get picture taken for PIV card (ID card). Remember to bring two forms of ID with you. The PIV office is in Building 1 on the first floor.
