VA Reaching Out to New Mexico OIF/OEF Veterans

VA Reaching Out to New Mexico's OIF/OEF Veterans - ALBUQUERQUE -   "No OIF/OEF Veteran Lost" is the goal of a new Transition Patient Advocate (TPA) at the VA hospital in Albuquerque. Scott Anderson recently joined the staff at the New Mexico VA Health Care System and has already begun his search for veterans who have come back from Iraq or Afghanistan after serving in Operation Iraqi Freedom or Operation Enduring Freedom. Each TPA is part of a new Transition of Care and Case Management Team that is based at the VA medical center closest to the veteran's home or best able to provide the services they require. Transition Patient Advocates meet with veterans, family members and may even travel wherever the veteran needs to go.  They help veterans with every conceivable problem, from filling out paperwork to obtaining an appointment to receive specialized clinical treatment.
