Description of Program:

The Farmington Community Based Outpatient Clinic (CBOC) is designed to give primary care to eligible veterans.

Services and Scope of Care:

• Blood Draws (by appointment only)
• EKGs (Electrocardiograms)
• Evaluations by Doctors
• Immunizations
• Nurse Clinics
• Routine Lab Tests
• Social Work Services
• X-Ray (Limited)

Hours of Operation:

Monday through Friday 8:00 a.m. - 4:30 p.m. (closed Saturday, Sunday and Federal holidays).

• No walk-in appointments are available at this facility. For appointments, call call the Clinic at (505) 326-4383/4384.
• To assist in your care and treatment, please bring all of your medications to your first appointment.
